My name is Abdullah Saleem, in 1987 while attending Lemoyne Owen College in Memphis,TN and also working at St. Jude Children's Research hospital I first began working with individuals with disabilities. Over the years while continuing to work in acute care nursing, I continued to have a passion for working with individuals with disabilities. In 2009, I made the transition to working full time with individuals with disabilities.
Seeing the necessity for more providers I made the commitment to become a licensed provider, so in late 2015, I began my journey to advocate and promote the self independence of all individuals with disabilities.

Terms of License:
To provide Home and Community-Based Services(HCBS) to persons with disabilities, and persons age 65 and older in any county in the state, subject to the standards of Minnesota Statutes, Chapter 245 A and Minnesota Statutes, Chapter 245D
Our 245D program is a licensed Home and Community Based Program, providing Basic Services in a manner that supports the individual with dignity
About our Clients:
Individuals will have the opportunity for the development and exercise of decision making, choice, and personal advocacy to support their desires of who will support them and how they will be supported by using a person-centered approach. The person receiving services and the guardian work together with staff to identify what is important to and for the individual which includes daily needs, and activities needed to accomplish their personal goals.
